After hours support

Our after-hours support team is focused on addressing urgent medical needs. They do not have access to your child's medical records or test results.

New patients

Welcome new patients! On your child's first visit, we ask you to arrive 30 minutes early to fill out registration and medical history forms. Please bring your insurance card, any medications your child is currently taking and any copayment or insurance deductible that applies. Having your child's previous medical records sent to us prior to their visit will aid the physician with their treatment.

Check-in

Please bring your insurance card with you for each appointment. All insurance copayments and deductibles are due at time of service. Copays will be collected before your child is seen. Please arrive 15 minutes before your child's scheduled appointment time to allow for the check-in process.
